LATE MORNING SESSION

Navigating the LTC/PAC for Meaningful Change

​

The forces impacting LTC/PAC throughout the COVID-19 pandemic have been unrelenting.  In this session, we will cover national efforts driving change including President Biden’s Reform Plan for Nursing Homes, AHCA Care for Our Seniors Act and the National Academies of Sciences, Engineering & Medicine Report on The National Imperative to Improve Nursing Home Quality.  We will discuss how to navigate these forces for meaningful change and share actionable steps to be prepared. 
 
Participants will: 

  • Learn the latest national reform efforts in progress across LTC/PAC.

  • Understand the impact that these national efforts may have on their practice. 

  • Gain an understanding of how to navigate the forces impacting LTC/PAC.

  • Explore actionable steps to be prepared for change and what can be done to drive meaningful change.

SNF Dollars ‘n $ense: Reimbursement, Compliance, and Revenue Recovery

​

Since early 2020, facilities have been diligently trying to manage the cumulative effects of the PHE. We are all very aware of the ongoing financial aftermath caused by significant expenditure increases to provide care, paying the price for staff turnover, increasing agency costs and reduced census.


As we continue to face ongoing COVID outbreaks, there are new looming challenges coming including CMSs push for tougher regulations, payment initiatives based on quality performance, new federal and state staffing mandates, continuing infection control surveys, rising Civil Monetary Penalty fines, laser focused reimbursement audits and now looming rate cuts outlined in the FY 2023 Proposed Rule.


As an industry we need to step back, regroup, take swift action and develop focused strategy to ensure fiscal health. This program will review these upcoming challenges, guide leadership to identify their own vulnerabilities and provide practical approaches to navigate through them.


Objectives:

  1. Review a process to identify potential areas of high risk.

  2. Review common PDPM pit falls and managed care complexities, including strategies for optimization.

  3. Understand the denials management process to minimize financial risk.

  4. Recognize the financial impact of a facilities quality metrics.

MID-MORNING SESSION

See it, Say it, Solve it - I. C.A.N!

​

The Infection Control Amplification in Nursing Centers or I C.A.N. is a shift coaching program designed to strengthen the infection control practices necessary to protect nursing home residents from coronavirus and other infectious diseases, by creating a network of shift coaches who provide just-in-time support to their peers – focused on hand hygiene, masking, and transmission-based precautions – and creating a feedback loop to give the infection preventionist added insight into day-to-day implementation of infection control practices. 


Ultimately, the program will help to model a culture where staff are empowered to “e it, Say it, Solve it-I. C.A.N! ” Developed by the Brown University Center for Long-Term Care Quality & Innovation, with funding from the Connecticut Department of Public Health, the program is spreading across the country.

 

Participants will discover:

  • The steps and reasons to adopt I CAN and the stories from those who have

  • The essential elements that lead to success

  • The tools and resources available to implement I CAN in your setting

Information Blocking Rule: What does this really mean for my SNF?

The first phase of the Information Blocking Rule took effect on April 5, 2021. The second phase takes effect on October 6, 2022. Both phases affect Post-Acute Care Providers.

 

In this session, we will review the basic information about this rule. However, most of the session will focus on the specific ways this rule will affect SNF Providers including the benefits.

 

Using scenario-based situations, we will focus on what needs to be reviewed in the SNF setting now, by which departments, and why. Join us for this enjoyable discussion and ask your questions along the way.

Team Leaders including dieticians, food service directors, nurse educators, administrators and speech pathologists will review the foundations for guiding their teams through IDDSI implementation, including the rationale for IDDSI framework and IDDSI-specific concepts, ideas and documentation to facilitate the transition from current diet levels to IDDSI levels.  We will discuss the regulatory expectations for diets in the SNF setting.  We will outline using IDDSI as a quality assurance performance initiative to achieve measurable QAPI outcomes throughout the implementation process.

Managing Behaviors Through Person-Centered Care Planning

​

The best approach to ensure quality care and avoid inappropriate psychoactive medications is person centered care planning. To meet federal quality and safety standards, skilled nursing facilities are required to assess each resident and develop an appropriate plan of care. CMS further mandates that medications may be considered for elderly residents with dementia only if non-pharmacological interventions are ineffective.

​

The misuse of psychotropic drugs is one of the most common and longstanding, but preventable, practices causing harm to nursing home residents today. This session will focus on the negative regulatory impact poor behavior management can have on care and quality outcomes, help identify causes and early indicators of behaviors and present effective approaches to minimize & effectively manage behaviors. These strategies will enhance quality of life, improve resident/family/staff satisfaction, positively impact your CMS Five Star, and much more.

 

After this program Participants will be able to:

  • Understand current regulatory guidelines related to behavior management.

  • Recognize early signs of behaviors and how to use person-centered care planning approaches effectively.
