Sarah M. Collins is an Adult/Geriatric Nurse Practitioner and holds a certification in Hospice and Palliative Care Advanced Practice Nursing. Her current role is Area Director for Amedisys Palliative Care and leads a team of Palliative Care Nurse Practitioners and Physician Assistants across six Palliative Care Practices in Massachusetts, Rhode Island, New Hampshire and Maine. She has 20 years of experience as a nurse practitioner and prior to becoming a Palliative Care Specialist she worked as a Nurse Practitioner in long term and post- acute care for 10 years.
Roxanne A. Dombrowski is a board-certified Family Nurse Practitioner and a 2014 graduate of Rivier University. She began her career in long-term care, where she developed a strong foundation in supporting patients with chronic and complex conditions.
Since joining Home Health & Hospice Care in 2016, Roxanne has served as a Palliative Care Nurse Practitioner, specializing in symptom management and quality-of-life support for patients with serious illness. In 2021, she earned her Advanced Certified Hospice and Palliative Nurse (ACHPN) credential, underscoring her commitment to excellence in end-of-life care.
Roxanne’s approach is rooted in compassion, active listening, and a deep respect for each patient’s values and wishes. She works closely with patients, families, and care teams to ensure care is personalized, dignified, and centered on what matters most.
