Delirium and Dementia Across Care Settings

Individuals travel across care settings depending on the care needed and who can provide that care - homecare, hospital, skilled nursing, long term care. In addition to a person living with dementia, an acute hospital stay can introduce delirium, which may complicate the care delivery for the individual. This presentation will provide an overview of dementia and delirium, education on how delirium affects a resident or patient living with dementia, and de-escalation techniques that can be widely used to diffuse situations.

Bernadette Campbell, BS, PT, CDP

Jane Cipriano, MHA, FACHE, CDP, is the Director of Post-Acute Services for the Concord Hospital Health System and the Skilled and Restorative Care program at Concord Hospital Franklin. She has over 25 years of experience in post-acute care, spanning independent and assisted living, skilled nursing and long term care settings. She has extensive experience building strong relationships and strategic partnerships with organizations across the state. Jane is a licensed Nursing Home Administrator in NH, a Fellow of the American College of Healthcare Executives (ACHE) and a Certified Dementia Practitioner.

Dana Bisson, M.S.N., R.N., C.D.P., C.P.A.F.H., is a Nurse Scientist with 25 years of experience caring for acutely ill patients, with a primary focus in the neurosciences. In 2019, she discovered a brief bedside intervention that catalyzed a paradigm shift in delirium care at her hospital. She now specializes in delirium and age-friendly care and leads an interdisciplinary team conducting novel research and developing, implementing, and modeling innovative clinical strategies to prevent and manage delirium.

Bernadette Campbell, BS, PT, CDP, is a physical therapist with over 44 years of experience across diverse clinical and leadership roles. She currently serves as Inpatient Rehabilitation Manager for Concord Hospital Laconia and Concord Hospital Franklin's Skilled and Restorative Care unit, where she oversees rehabilitation services focused on optimizing patient outcomes and quality of care. Her career includes owning and operating a private physical therapy clinic, serving as Director of Rehabilitation and Deputy Director of Medical for the New Hampshire Department of Corrections, and working in assisted living, memory care, and skilled nursing settings. Bernadette is especially passionate about the geriatric population, a commitment that has shaped both her professional path and personal mission. She is dedicated to ensuring older adults receive compassionate, high-quality care in safe and supportive environments.
